In accordance with Article 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (Official Journal of the European Union, L 119, 04.05.2016, p. 1), hereinafter referred to as „GDPR”, we inform that:

- 2. Purpose of personal data processing: functioning of the system of reporting cases related to safety.
- 3. Recipient of personal data: authorized employees of the Upper Silesian Aviation Group.
- 4. You have the right to access and correct content of the data.
- 5. Data is collected on the basis of Article 6 (1) (c) of GDPR as a result of ADR.OR.D.030 of REGULATION (EU) NO 139/2014 of 12 February 2014 laying down requirements and administrative procedures related to aerodromes pursuant to Regulation (EC) No 216/2008 of the European Parliament and of the Council.

Your data will be processed over the period of 5 years as a result of ADR.OR.D.030 of REGULATION (EU) NO 139/2014 of 12 February 2014 laying down requirements and administrative procedures related to aerodromes pursuant to Regulation (EC) No 216/2008 of the European Parliament and of the Council.
- 7. Provision of personal data is voluntary. Failure to provide personal data will result in the inability to confirm the acceptance of the notification for the notifier and provide feedback about the result of the analysis of the event.
- 8. You have the right to remove or limit the processing of personal data, the right to appeal against processing of personal data, and the right to transfer data.
- 9. Profiling is not applicable.
- 10. Intention of transferring the Data to a third country or international organization is not applicable.
